This study aimed to describe the implementation of nursing care for An. N with respiratory failure and aspiration pneumonia in the Pediatric Intensive Care Unit (PICU) of RSUD dr. Zainoel Abidin Banda Aceh. This study used a case study design with a nursing process approach including assessment, nursing diagnosis, planning, implementation, and evaluation. Data were collected through observation, physical examination, medical record review, and collaboration with the healthcare team. The nursing assessment identified six nursing diagnoses, namely impaired gas exchange, ineffective airway clearance, nutritional deficit, risk for hypothermia, risk for developmental delay, and risk for aspiration. Nursing interventions were implemented through respiratory monitoring, oxygen therapy support using Continuous Positive Airway Pressure (CPAP), airway management, nutritional support, thermal protection, developmental care, and aspiration prevention. The evaluation showed improvements in oxygen saturation, respiratory effort, airway patency, and feeding tolerance. Risk factors for hypothermia, developmental disorders, and aspiration were successfully prevented during hospitalization. Comprehensive nursing care contributed to maintaining patient stability and supporting the recovery process during intensive care.
