Asuhan Keperawatan Komunitas pada Agregat Lansia dengan Hipertensi Melalui Edukasi Deteksi Dini Stroke di Desa Lam Asan Kecamatan Darussalam Kabupaten Aceh Besar

This study aimed to describe community nursing care for older adults with hypertension, focusing on improving knowledge of early stroke detection in Lam Asan Village, Darussalam Subdistrict, Aceh Besar Regency. Assessment was conducted using the community-as-partner concept through windshield surveys, Focus Group Discussions (FGDs), and interviews with community health cadres and village midwives. The nursing diagnosis established was ineffective health self-management. Intervention planning was developed using the Nursing Outcomes Classification (NOC) and Nursing Interventions Classification (NIC), with a focus on the Wheels of Intervention approach and health education. Implementation was carried out through education using the BEFAST method for 45 minutes with PowerPoint presentations and leaflets as educational media. The evaluation was conducted using pre- and post-test assessments with 10 older adults with hypertension. The results showed a significant increase in knowledge, with the mean score for early stroke detection increasing from 3.8 to 9.2. It can be concluded that BEFAST-based health education was effective in improving knowledge among older adults with hypertension. Therefore, community-based interventions that involve the community as a partner and the Wheels of Intervention approach are recommended as promotive and preventive strategies to prevent stroke complications.
