Asuhan Keperawatan pada Lansia Tn. M dengan Post Stroke di Gampong Geuceu Kayee Jato Kecamatan Banda Raya Kota Banda Aceh

Abstract

Stroke is one of the most common chronic diseases among older adults and can lead to various physical and psychological health problems. This scientific paper aims to report the implementation of nursing care for an elderly patient (Mr. M) with a post-stroke condition in Geuceu Kayee Jato Village, Banda Raya District, Banda Aceh City. The assessment findings revealed that the patient had a history of hypertension and had experienced a stroke one month earlier, with complaints of knee pain and weakness in the left upper and lower extremities. The nursing diagnoses established included chronic pain, disturbed sleep pattern, impaired physical mobility, ineffective health management, and risk of falls. Nursing interventions consisted of warm compress therapy, Qur’anic murottal therapy, Range of Motion (ROM) exercises, education on hypertension and the DASH diet, and fall prevention education. Evaluation results showed a reduction in pain intensity from 3 to 2 on the Numeric Rating Scale (NRS), improved sleep quality, and increased knowledge regarding ROM exercises, hypertension management, the DASH diet, and fall prevention. The diagnoses of chronic pain, ineffective health management, and risk of falls were resolved, while disturbed sleep pattern and impaired physical mobility were partially resolved. It is recommended that family members and healthcare providers continue providing education on hypertension and stroke care for older adults.
