The prevalence of adenomyosis at Dr. Zainoel Abidin Hospital, Banda Aceh, during 2021–2022 was 1.60%. Effective management for women with adenomyosis is hysterectomy. The aim of this case study is to implement nursing care for Mrs. I following hysterectomy with an indication of adenomyosis in the Arafah 3 ward of RSUDZA Banda Aceh. The nursing diagnoses identified include acute pain, constipation, and risk of infection. Interventions provided for the diagnosis of acute pain included finger-holding relaxation therapy and Qur’anic murotal therapy. For constipation, interventions included warm water consumption combined with papaya intake. For the risk of infection, the intervention involved encouraging increased nutritional intake, particularly protein-rich foods. The evaluation results during the course of care showed that, for the diagnosis of acute pain, the patient experienced a reduction in pain and reported increased comfort. The diagnosis of constipation was resolved, indicated by bowel peristalsis exceeding five times per minute and regular defecation. Regarding the risk of infection, the wound area showed no signs of redness or cyanosis, and pain levels decreased. The findings of this study indicate that finger-holding relaxation therapy can reduce postoperative pain following hysterectomy, with patients reporting greater relaxation and comfort.
