Asuhan Keperawatan pada Tn.A dengan Traumatic Intracerebral Hemorrhage (t-ICH) di Instalasi Gawat Darurat Rumah Sakit dr. Zainoel Abidin Kota Banda Aceh

This case study aims to describe nursing care for patients with traumatic intracerebral hemorrhage (t-ICH) in the Emergency Department (ED) at dr. Zainoel Abidin Regional General Hospital in Banda Aceh. The method used is a descriptive case study with a nursing process approach. The nursing problems identified in this case include decreased intracranial adaptive capacity, acute pain, and risk of shock. Interventions implemented include managing increased intracranial pressure, pain management, and shock prevention through monitoring neurological status, vital signs, oxygen administration, and collaboration on medical therapy. The evaluation results during monitoring in the resuscitation room showed that the patient's condition was relatively stable with a Glasgow Coma Scale (GCS) score of 10 (E2M5V3), blood pressure of 137/88 mmHg, pulse rate of 81 beats per minute, respiratory rate of 22 breaths per minute, and oxygen saturation of 99% with oxygen therapy using a simple mask at 8 L/min. The patient still has active bleeding in the cranial area and is planned for craniotomy. It is recommended that healthcare providers in the emergency department continue to improve the quality of nursing care for patients with traumatic intracerebral hemorrhage through close neurological and hemodynamic monitoring, as well as appropriate interventions to prevent further complications.
