Asuhan Keperawatan pada Pasien dengan Neglegted of the Right Neck Femur di Ruang Orthopedi Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh

Abstract

This case study aimed to provide nursing care for Mrs. J, diagnosed with a neglected fracture of the right femoral neck, in the Orthopedic Ward of dr. Zainoel Abidin General Hospital, Banda Aceh. The assessment revealed that the patient experienced pain in the right hip, with the following characteristics: P: Hip fracture, Q: Sharp pain, R: Radiating to the waist, S: 3 on the Numeric Rating Scale (NRS), and T: approximately 10–15 minutes (intermittent). Additional findings included shortness of breath, limited range of motion, postoperative sutured wound, inability to sleep, and frequent awakenings. Identified nursing problems included acute pain, ineffective airway clearance, impaired physical mobility, impaired tissue integrity, and disturbed sleep patterns. The interventions provided consisted of cold compress therapy, deep breathing and relaxation exercises, semi-Fowler’s position, effective coughing techniques with chest physiotherapy, nebulizer administration, Range of Motion (ROM) exercises, sterile wound care, and recitation therapy of the Holy Qur’an. Nursing evaluation indicated that pain decreased to a score of 3 (NRS), the patient no longer used accessory respiratory muscles, physical mobility improved as evidenced by increased range of motion and the ability to perform passive ROM with family assistance, postoperative wound healing progressed well, and the patient was able to sleep as usual. The recommendation is to avoid excessive physical activity, especially activities that involve the knee joint.
