Asuhan Keperawatan pada Pasien dengan Stroke Hemoragik di RSUD dr. Zainoel Abidin Banda Aceh

Abstract

Hemorrhagic stroke is a condition of intracerebral bleeding that can cause neurological impairment and decreased consciousness, thereby requiring comprehensive nursing care. This scientific paper employed a case study approach involving a patient with hemorrhagic stroke in the Mina 1 ward at dr. Zainoel Abidin Regional General Hospital, Banda Aceh, for seven days from August 17–23, 2025. Assessment findings revealed decreased consciousness with a Glasgow Coma Scale (GCS) score of E2VxM4, blood pressure of 124/63 mmHg, secretion accumulation, ineffective cough, oxygen therapy via nasal cannula, right-sided hemiplegia, aphasia, nasogastric tube insertion, and swelling in the mandibular fracture area, with a leukocyte count of 13.77 × 10³/mm³. Nursing problems included decreased intracranial adaptive capacity, ineffective airway clearance, impaired physical mobility, impaired verbal communication, risk of aspiration, and risk of infection. Nursing Interventions consisted of neurological and vital sign monitoring, maintaining a 30° head-up position, airway suctioning, oxygen therapy, passive range of motion (ROM) exercises, extremity elevation, Eye-Blink AAC therapy, enteral feeding through a nasogastric tube, hand hygiene, and collaborative antibiotic administration. Evaluation showed improvement in consciousnesss, as indicated by an increase in the GCS score of E3VxM4, reduced secretion production, and absence of aspiration or infection, although no improvement was observed in muscle strength or verbal communication ability.
