Placenta percreta is the most severe form of the placenta accreta spectrum and can lead to massive hemorrhage and serious obstetric complications, thus requiring comprehensive management, including nursing care following a cesarean section. This research aims to describe the implementation of nursing care for Mrs. R with placenta percreta post–cesarean section in the Arafah 2 ward of Dr. Zainoel Abidin Regional General Hospital, Banda Aceh. The method used was a case study with a nursing process approach that includes assessment, nursing diagnosis, planning, implementation, and evaluation stages. The results of the nursing care implementation showed four main nursing diagnoses, namely risk for bleeding, acute pain, ineffective breastfeeding, and risk of infection. Nursing interventions were carried out comprehensively through monitoring vital signs and bleeding, pain management, infection prevention, breastfeeding support, health education, and collaboration with medical personnel. The evaluation showed that the patient’s condition improved, with the problems of risk for bleeding, acute pain, ineffective breastfeeding, and risk of infection being partially resolved during the hospitalization period. In conclusion, the implementation of evidence-based practice plays an important role in maintaining the patient’s hemodynamic stability and supporting the recovery process in patients with placenta percreta post–cesarean section.
