Asuhan Keperawatan pada Pasien dengan Stroke Iskemik: Studi Kasus

This case study aimed to provide nursing care for Mr. J, who had an ischemic stroke, in the Mina 1 Ward of dr. Zainoel Abidin Regional General Hospital in Banda Aceh. Nursing care was implemented for six days, from August 6–11, 2025. The assessment results identified seven nursing problems: decreased intracranial adaptive capacity, risk of ineffective cerebral perfusion, impaired physical mobility, unstable blood glucose levels, impaired swallowing, impaired verbal communication, and risk of falls. The interventions provided included maintaining the head of the bed at a 30° elevation, monitoring blood pressure, pulse, and respiration, Range of Motion (ROM) exercises accompanied by the use of a rubber ball, periodic monitoring of blood glucose levels, swallowing therapy, AIUEO speech therapy, and education on fall risk prevention. The evaluation results after six days of nursing care showed that two nursing problems were resolved (impaired swallowing and risk of falls), four nursing problems were partially resolved (decreased intracranial adaptive capacity, risk of ineffective cerebral perfusion, impaired physical mobility, unstable blood glucose levels), and one nursing problem remained unresolved (impaired verbal communication). Patients and their families are expected to continue performing the recommended exercises to improve muscle strength and range of motion, as well as to control risk factors for recurrent stroke.
